To apply for credit with Industrial Lumber & Plywood, Inc., please do the following:

• Print the Application of your choice (Open Credit or C.O.D.)
• Print the Fax & Email Authorization Form.
• Print the Certificate of Exemption Form if you are tax exempt.
• Fill out the forms in full and mail or fax to:

Open Credit Applications will take up to 1 week to process. A letter will be sent to confirm the approval status of your application.

C.O.D. applications will take up to 2 days to process. A letter will be sent to confirm the approval status of your application.
